SUMMER CAMP REGISTRATION & PARTICIPATION AGREEMENT
This Agreement is entered into between Warrior Martial Arts (“School”) and the Parent/Legal Guardian of the minor child listed below.

SECTION 1 — ENROLLMENT
I am enrolling my child in the Warrior Martial Arts Summer Camp (“Dunellen's BEST Summer Camp”).
SECTION 2 — ASSUMPTION OF RISK & WAIVER OF LIABILITY
I understand that participation in martial arts, sports, games, fitness activities, obstacle courses, inflatables, and other physical activities involves inherent risk of injury.
I voluntarily assume all risks on behalf of my child and certify my child is physically and mentally capable of participating.
I release, waive, and hold harmless Warrior Martial Arts, its owners, staff, contractors, and affiliates from any claims or liability for injury, loss, or damage arising from participation, except where prohibited by law.
SECTION 3 — MEDICAL AUTHORIZATION
In the event of an emergency, I authorize Warrior Martial Arts to obtain medical treatment for my child if I cannot be reached.
SECTION 4 — PAYMENT & BILLING AUTHORIZATION
I authorize Warrior Martial Arts to charge my payment method on file for:
• Summer camp tuition
• Registration fees
• Merchandise purchases
• Late pickup fees
• Replacement items
• Snacks or drinks (if selected)
I understand all payments are non-refundable and non-transferable.
If a payment fails due to insufficient funds or expired card, a $15 late fee may be applied.
All payments must be made before June 13, 2026
SECTION 5 — ATTENDANCE, ABSENCES & REFUNDS
No refunds or credits will be issued for missed days.
No refunds on single-payment memberships or registration fees.
SECTION 6 — HEALTH & ILLNESS POLICY
My child may not attend camp if sick and must be symptom-free for 24 hours before returning.
SECTION 7 — DROP-OFF & PICK-UP
I understand daily pick-up deadlines apply.
Late pickup fees may be charged per minute after designated closing time.
Approved adults must present government-issued ID for pickup.
SECTION 8 — FIELD TRIPS
I understand certain field trips may be optional and may require additional fees.
My child must wear an approved Warrior Summer Camp shirt on field trip days.
No refunds for missed field trips.
SECTION 9 — REQUIRED ITEMS
My child must bring:
• Bagged lunch
• Two snacks
• Water bottle
Replacement water bottles or snacks may be charged to my account.
SECTION 10 — PHOTO & VIDEO RELEASE
I grant Warrior Martial Arts permission to photograph and record my child for marketing and promotional purposes.
I understand no compensation will be provided.
SECTION 11 — BEHAVIOR & DISCIPLINE
My child must follow all camp rules and staff instructions.
Warrior Martial Arts reserves the right to suspend or dismiss any child for unsafe, disruptive, or inappropriate behavior without refund.
SECTION 12 — ELECTRONIC DOCUMENT CONSENT
I consent to electronic records, signatures, and communications.
